The Kwara State College of Health Technology (CHT) Offa admission form is out for 2024/2025 academic session. The admission form for the 2024/2025 session is now available, and registration is open to students with and without JAMB scores (depending on the programme).
Admission into Kwara State College of Health Technology Offa commences on Wwednesday, 14th February, 2024 and is open for National Diploma (ND), Higher National Diploma (HND), and Certificate programmes.

Requirements for Admission
- ND applicants must possess a minimum of five credits in English, Mathematics, Biology, Chemistry and Physics in not more than two sittings, including WAEC, NECO, or NABTEB
- WAEC and NECO can be combined but NABTEB must be in one sitting
- Candidates with awaiting result can apply
- HND and ND applicants must have a 2024 JAMB result with a minimum score of 100 and above cut off mark.
- HND applicants must have a minimum of lower credit in ND
- Certificate Programme applicants must have O’ Level Credits in English. Mathematics, Biology and any other two passes in science subjects.
Method of Application
Interested candidates should visit the Offa Health Tech admission application e-portal at https://offahealthtech.com/eportal and click on Apply for 2024/2025 Application Form on the menu section and complete the application form.
Print out thr CHT Offa acknowledgement/examination slip, completed application form, and referee form.
Applicants will be required to make a payment of N15,000 for the application fee during the registration. During the registration, applicants should only make that payment for application form through the college e-portal.
Candidates can also come to the school campus for registration. After registration, applicant should print out the Application Slip.
Candidates are to attend the screening in person with their:
- Originals of relevant credentials O’ level or A’ Level results
- Acknowledgement/examination Permit Slip
- Completed Application Form
- Completed referee form
- Photocopies of all results presented will be submitted at the center
- Photocopy of indigene letter
- Certificate of birth/declaration of age
Candidates will be contacted via SMS on their phone for the date, time and venue of screening.
The portal for the 2024 academic session will open for registrations on the specified date on the Application Slip. The 2024 closing date for registration is indicated on the notification slip.
