UCH School of Health Information Management Ibadan 2024/2025 Admission Form

The University College Hospital (UCH) School of Health Information Management (SHIM), Ibadan 2024/2025 admission form is out. Admission is ongoing into Higher National Diploma, National Diploma, and Professional Diploma Programmes.

School of Health Information Management UCH Requirements 2024/2025

  • Applicant must have obtained the minimum of 5 credits at not more than two sittings in SSCE, GCE, WAEC, or NECO from the same examination body in English Language, Mathematics, Biology, Physics and Chemistry
  • ND applicants must score 150 and above in 2024 UTME
  • HND applicants must present their O’ level Result, ND or Professional Diploma in HIM, License Certificate, and Letter of completion of IT

How to Apply for School of Health Information Management UCH in 2024

Interested ND and HND applicants are to pay a non-refundable application and screening fee (as indicated on the application form for 2024) into the TSA ACCOUNT (TSA/UCH/SHIM/ADMISSION FORM) in any commercial bank.

Both ND and HND applicants will also pay another fee of N2,000 for access code and CBT exercise into the TSA ACCOUNT (UCH/Information Technology Revolving) in any commercial bank. The applicant must convert his/her bank teller into a receipt at the Cash Office of the Finance and Accounts Department, UCH, and forward the photocopy to the Administrative Office of the School.

Applicants MUST make sure that their email address is valid to be able to receive the access code from the school Administrative office and apply online at https://portal.uch-ibadan.org.ng.

After the registration, the applicant should print out their completed online application form and examination card and submit to the Administrative Office of the School.

The screening exercise for the ND programme and HND entrance examination will be sent to successful candidates via their email address. Only successful applicants will receive an oral interview invitation at a later date through text message.
